Not All Anxiety Is the Same
Anxiety is one of the most common co-occurring experiences in both ADHD and autism, with studies suggesting rates of 40–60% for clinically significant anxiety across both populations. But neurodivergent anxiety often looks and feels different from the generalised anxiety disorder described in textbooks — and understanding those differences is essential for managing it effectively.
If generic anxiety advice hasn't worked for you, the problem probably isn't that you're not trying hard enough. It's that the advice doesn't account for how your nervous system actually works.
Autistic anxiety: the unpredictability problem
A significant driver of autistic anxiety is uncertainty. Not the philosophical kind — the moment-to-moment kind. Will the train be on time? Will the meeting agenda change? Will someone ask an unexpected question? Will the restaurant be louder than expected?
Neurotypical brains handle uncertainty by defaulting to "it'll probably be fine." Autistic brains often default to "I need to prepare for every possible outcome." This isn't irrational — it's a predictive processing system running at maximum capacity because past experiences have taught it that unexpected things are often overwhelming.
ADHD anxiety: the consequence spiral
ADHD anxiety often centres on consequences — of forgetting something, of being late, of missing a deadline, of saying the wrong thing. This is partly learned (years of actually forgetting things and facing consequences) and partly neurological (impaired working memory means you genuinely can't trust yourself to remember, which creates a legitimate reason for vigilance).
The result is a constant background hum of worry that something important is slipping through the cracks. It's exhausting not because the individual worries are big, but because the monitoring never stops.
Interoceptive confusion
Both autistic and ADHD people can have difficulty interpreting internal body signals — a phenomenon called interoceptive difference. Your heart rate increases, your stomach tightens, your breathing changes... and your brain interprets these as threat, triggering an anxiety response. But the original signal might have been hunger, dehydration, excitement, or even needing the bathroom.
This means neurodivergent people can experience anxiety that has no psychological trigger at all — it's a body signal being misread. Standard cognitive approaches ("challenge the thought") fail here because there's no thought to challenge. The anxiety is physiological.
Strategies That Target Neurodivergent Anxiety
Reduce unpredictability
This is the single most effective anxiety intervention for many autistic people:
- Visual schedules — knowing what's coming next, even in broad strokes, calms the predictive system. A simple daily plan on paper or a whiteboard reduces the "unknown" category.
- Preparation rituals — before social events, crowded spaces, or meetings, review what to expect. Where will you sit? How long will it last? Is there an exit plan if it becomes too much?
- Communication scripts — having pre-prepared responses for common social situations reduces the cognitive load of real-time improvisation. This isn't inauthentic; it's practical accommodation.
- Transition warnings — if you share a space with others, ask for advance notice before changes. "We're leaving in 15 minutes" is infinitely more manageable than "We need to go now."
Ground through the body
When anxiety is driven by interoceptive confusion or sensory overwhelm, cognitive strategies alone won't cut it. Somatic (body-based) approaches work by directly downregulating the nervous system:
- 5-4-3-2-1 grounding — name 5 things you can see, 4 you can touch, 3 you can hear, 2 you can smell, 1 you can taste. This pulls your attention out of threat-scanning and into the present moment.
- Stimming with purpose — rocking, hand-flapping, squeezing a stress ball, or chewing something textured aren't just comforting habits; they provide rhythmic proprioceptive input that directly calms the autonomic nervous system.
- Cold water reset — splashing cold water on your face or holding ice cubes activates the dive reflex, which slows heart rate and shifts the nervous system out of fight-or-flight. This is one of the fastest anxiety interventions available.
- Bilateral stimulation — alternating tapping on your knees, walking, or even moving your eyes side to side can reduce the intensity of anxious feelings. This is the mechanism behind EMDR therapy, but the basic version is available to anyone.
Build an anxiety early warning system
Track which situations, environments, and patterns precede your anxiety spikes. Your tracking data can reveal triggers you might not consciously recognise:
- Does anxiety rise after social situations? After sensory exposure? After poor sleep?
- Are certain days of the week worse? (Often Monday/Sunday — anticipatory anxiety about the week ahead.)
- Does anxiety correlate with masking load?
- Is there a time-of-day pattern? (Many neurodivergent people experience peak anxiety in the evening when the structure of the day dissolves.)
Once you know your patterns, you can intervene earlier — before anxiety becomes overwhelming.
Separate the signal from the noise
When anxiety strikes, run through a quick checklist:
- Am I hungry or thirsty?
- When did I last move my body?
- Is my sensory environment uncomfortable?
- Have I been masking for an extended period?
- Am I interpreting a body sensation as threat?
If any of these are true, address the underlying need first. You may find the anxiety resolves without any psychological intervention at all.
Scheduling and structure as medication
For ADHD anxiety specifically, external structure reduces the monitoring burden:
- Use reminders, alarms, and automated systems for anything that currently requires mental effort to remember.
- Keep a single, reliable capture system (notebook, app, whatever you'll actually use) and trust it. The goal is to stop holding things in working memory.
- Break large tasks into small, concrete next steps. "Write report" generates anxiety. "Open document and write the first three sentences" doesn't.
When to Seek Additional Support
If anxiety is significantly impairing your daily functioning — avoiding situations, persistent physical symptoms, inability to get through routine tasks — it's worth discussing with a healthcare provider who understands neurodivergence. Not all therapists are equipped for this; look for practitioners who explicitly work with autistic or ADHD adults. CBT can be effective when adapted for neurodivergent cognition, and medication (both for anxiety specifically and for the underlying ADHD/autistic support needs) can provide the baseline stability that makes other strategies possible.
Anxiety in neurodivergent people isn't a personal failing. It's a signal from a nervous system that's working overtime in a world that wasn't designed for it.
