The Missing Thirst Signal
If you regularly forget to drink water until you have a headache, or you reach the end of the day and realise you've had nothing but coffee — you're not careless. You're likely experiencing interoceptive differences, which are common in both autism and ADHD.
Interoception is the sense that monitors internal body states: hunger, thirst, temperature, bladder fullness, heart rate, and more. In neurodivergent people, these signals can be muted, delayed, or confusing. You might not feel thirsty until you're already significantly dehydrated. You might not recognise hunger until you're dizzy. You might notice your heart racing without connecting it to the four coffees you had before noon.
This isn't a lifestyle choice to fix with motivation. It's a sensory processing difference that requires external systems to compensate.
What mild dehydration actually does
Even 1–2% dehydration (before you feel thirsty) measurably impairs:
- Working memory and attention — precisely the functions already challenged by ADHD
- Mood — dehydration increases anxiety, tension, and fatigue
- Sensory tolerance — lower hydration correlates with increased sensitivity to noise, light, and touch
- Cognitive processing speed — tasks take longer and errors increase
For neurodivergent people operating with already-limited executive function and sensory capacity, dehydration quietly makes everything harder without providing an obvious signal for why.
Practical Hydration Strategies
The key principle: don't rely on feeling thirsty. Build hydration into external systems.
Routine anchoring
Attach water intake to existing habits that already happen reliably:
- Drink a full glass when you take any medication or supplements
- Keep water next to your toothbrush and drink before/after brushing
- Pair every meal with a glass of water
- Drink water immediately when you sit down at your workspace
- Have water when you take a sensory or movement break
Environmental cues
- Visible water bottle — if it's out of sight, it doesn't exist. Keep it on your desk, next to your keyboard, in your direct line of vision.
- Marked bottles — water bottles with time markings (drink to here by 10 AM, here by noon) externalise the tracking.
- Multiple stations — keep water in every room you spend time in. Reducing the distance between you and hydration reduces the executive function cost of getting a drink.
Gentle reminders
- Phone alarms at regular intervals (every 60–90 minutes)
- Smart watch tap reminders
- Pair with an existing timer system (if you're already using Pomodoro, drink water at each break)
Make it appealing
Neurodivergent brains are more likely to engage with things that have some sensory interest:
- Add flavour — lemon, cucumber, berries, or sugar-free squash
- Temperature matters — some people only drink cold water; others prefer room temperature
- The vessel matters — a water bottle you enjoy holding or drinking from removes friction
- Sparkling water can provide the sensory novelty that makes drinking feel more engaging
The Caffeine Question
Caffeine has a complicated relationship with neurodivergence. Many ADHD people describe caffeine as essential for functioning — and there's a neurological reason for this. Caffeine increases dopamine availability in the prefrontal cortex, temporarily improving focus, motivation, and task initiation. For unmedicated ADHD, caffeine can function as mild self-medication.
The problem isn't caffeine itself. It's the pattern that develops.
The caffeine-sleep cycle
- Poor sleep reduces focus and energy
- You drink more caffeine to compensate
- Caffeine (especially after midday) impairs sleep architecture
- You sleep poorly again
- You drink more caffeine...
Caffeine has a half-life of 5–6 hours, meaning half of the caffeine from a 2 PM coffee is still in your system at 8 PM. For people who are already sensitive to stimuli (as many autistic people are), even caffeine consumed earlier can disrupt sleep quality without preventing sleep onset — you fall asleep, but the restorative deep sleep phases are reduced.
Finding your balance
The goal isn't necessarily zero caffeine. It's understanding your threshold and timing:
- Morning caffeine is generally fine — consumed before 10–11 AM, it's largely cleared by evening.
- Set a cutoff — 1 PM is a common recommendation, but you may need earlier depending on your sensitivity. Track your sleep quality against caffeine timing to find your personal limit.
- Monitor total intake — it's easy for intake to creep up, especially during stressful or high-demand periods. If you're above 3–4 cups equivalent daily and sleep is suffering, reducing by one cup per week is a sustainable approach.
- Watch for anxiety amplification — caffeine activates the sympathetic nervous system. If you're already running high on anxiety or sensory overload, caffeine will amplify both. On high-anxiety days, switching to decaf or tea (lower caffeine, plus the calming effects of L-theanine) may be worth it.
ADHD medication and caffeine
If you take ADHD stimulant medication, be cautious about stacking caffeine on top. Both increase dopamine and norepinephrine, and together they can produce anxiety, jitteriness, increased heart rate, and disrupted sleep. Discuss your caffeine intake with your prescriber — some people need to significantly reduce caffeine when starting or adjusting medication.
Tracking the Connection
Your daily entries track both hydration and caffeine intake alongside sleep, mood, focus, and sensory load. Look for these patterns:
- Does focus improve on higher-hydration days?
- Does sleep quality drop when caffeine intake is elevated?
- Does anxiety correlate with caffeine timing?
- Does sensory tolerance decrease on low-hydration days?
The data often makes the case more compellingly than any list of advice. When you can see that your worst sleep nights consistently follow your highest caffeine days, or that your best focus days coincide with good hydration, the motivation to change follows naturally.
Small adjustments compound. Replacing one afternoon coffee with water and setting a single hydration reminder can shift your baseline within a couple of weeks. You don't need to overhaul everything — you just need to close the gaps that interoception isn't covering.
